AFFILIATE REGISTRATION — AGREEMENT

By submitting this Affiliate Registration Form, I acknowledge that I have read, understood, and agree to the following terms and conditions:

1. COMMUNITY SERVICE MEMBERSHIP (CSM)

• I understand that Community Service Membership is intended for individuals or groups who voluntarily serve as full-time workers in support of the mission and vision of PITMC.

• I agree to uphold the values, policies, ethical standards, and objectives of PITMC.

• I understand that CSM provides opportunities to participate in volunteer service, community programs, organizational activities, and other approved PITMC initiatives.

• I understand that CSM members may have access to applicable free webinars, seminars, workshops, and organic training programs.

• I acknowledge that membership privileges, voting rights, nomination rights, and organizational participation are subject to PITMC policies and applicable requirements.

• I understand that the standard CSM registration fee is ₱0.00, subject to the applicable terms of the organization.

• I acknowledge the stated membership validity of three (3) years, unless otherwise amended or terminated in accordance with PITMC policies.

2. REGULAR MEMBERSHIP (RM)

• I understand that Regular Membership is intended for individuals who wish to maintain an ordinary membership primarily for continuing learning, personal development, networking, and exploration of transformational teachings.

• I understand that RM members may access applicable free online organic training programs and may participate in volunteer activities, subject to PITMC policies.

• I understand that participation in volunteer activities does not automatically change my membership category to Community Service Membership.

• I acknowledge the applicable membership benefits, including digital membership certification, website access, applicable discounts, and opportunities for upgrading to mentor or freelance trainer status, subject to evaluation and approval.

• I understand that IDs, uniforms, hardcopy certificates, and other physical materials may require additional fees.

• I acknowledge that the standard registration fee is ₱1,000.00.

• I understand that Regular Membership is valid for one (1) year, subject to renewal.

• I understand that promotional discounts or special registration arrangements may be offered by PITMC subject to applicable promotional terms.

3. INDIVIDUAL AFFILIATION — PROFESSIONAL OR NON-PROFESSIONAL (I.M.)

• I understand that Individual Affiliation is intended for professionals, non-professionals, associates, and practitioners seeking formal affiliation with PITMC.

• I understand that this affiliation may provide opportunities for professional development, specialized training, certification pathways, research, publications, workshops, mentoring, and community-based programs.

• I acknowledge that participation in PITMC research, publications, projects, workshops, and other activities may be subject to qualifications, evaluation, and organizational approval.

• I understand that qualified affiliates may have opportunities to serve as trainers, facilitators, program assistants, researchers, or other contributors, subject to evaluation and approval.

• I acknowledge the applicable benefits, including access to specialized and advanced organic training programs, applicable training discounts, professional and practitioner networking, directory inclusion, and opportunities for certification or upgrading to the Mentor/Trainer Program.

• I understand that IDs, uniforms, hardcopy certificates, and other physical materials may carry additional fees upon request and approval.

• I acknowledge that the standard registration fee is ₱2,000.00.

• I understand that Individual Affiliation is valid for two (2) years, subject to renewal and applicable PITMC policies.

4. COMPANY OR ORGANIZATION AFFILIATE (COA)

• I understand that Company or Organization Affiliate registration is intended for companies, institutions, training centers, organizations, and other entities seeking formal collaboration or affiliation with PITMC.

• I understand that affiliation may support joint programs, educational activities, training, research, community outreach, professional development, and other initiatives aligned with PITMC's mission and vision.

• I agree that any joint activity, program, project, partnership, or use of PITMC's name, logo, materials, or resources shall be subject to prior coordination and approval by authorized PITMC representatives.

• I understand that the specific scope of collaboration, responsibilities, deliverables, resources, recognition, and other arrangements may be defined through a separate partnership agreement, memorandum of understanding, memorandum of agreement, or other applicable document.

• I agree that my organization will observe applicable PITMC policies, ethical standards, safeguarding requirements, and organizational guidelines when participating in joint activities.

• I understand that any registration fee, project cost, training fee, or other financial arrangement applicable to the affiliation shall be communicated and agreed upon by the parties before implementation.

5. CUSTOMIZED PROJECT (CP)

• I understand that a Customized Project is a special project-based engagement designed for individuals, professionals, companies, organizations, institutions, or community groups requiring a customized program or service.

• I understand that the project may involve customized training, research, community initiatives, partnerships, capacity building, outreach, or other specialized activities.

• I acknowledge that each project may have specific objectives, scope, deliverables, timelines, resources, responsibilities, and conditions agreed upon by the parties.

• I understand that customized projects are outside the standard PITMC membership or affiliation categories and therefore may require separate project terms and agreements.

• I agree that project implementation shall begin only after the scope, responsibilities, deliverables, fees or funding arrangements, and other applicable conditions have been properly discussed and approved.

• I understand that the applicable project fee or financial arrangement shall depend on the approved scope and requirements of the customized project.

• I acknowledge that a separate proposal, quotation, project agreement, memorandum of agreement, or similar document may be required when applicable.

GENERAL TERMS AND CONDITIONS

• I certify that the information I provide in this registration is true, accurate, and complete.

• I agree to comply with the mission, vision, core values, Code of Ethics, policies, procedures, and legitimate directives of PITMC.

• I understand that registration or affiliation does not automatically guarantee appointment, employment, certification, training-facilitator status, leadership position, project approval, or any other special designation unless separately approved by PITMC.

• I understand that certain privileges, certifications, discounts, training opportunities, leadership roles, and organizational positions may be subject to qualifications, evaluation, availability, and approval.

• I agree to conduct myself with integrity, respect, compassion, confidentiality, accountability, social responsibility, and continuous growth, consistent with PITMC's Code of Ethics.

• I agree to protect confidential organizational, member, client, research, and project information obtained through my participation in PITMC.

• I understand that PITMC operates as a non-stock, non-profit organization and that its programs and resources are intended to advance education, research, community development, outreach, and related organizational purposes.

• I understand that PITMC may receive support through membership fees, donations, grants, sponsorships, fundraising activities, and institutional partnerships, as applicable to its programs and operations.

• I understand that membership, affiliation, or participation may be suspended, terminated, or otherwise reviewed in accordance with applicable PITMC policies and procedures.

• I acknowledge that PITMC encourages collaboration with government agencies, educational institutions, private organizations, and community partners in support of its programs and advocacy.

• By submitting this registration, I confirm my voluntary acceptance of the applicable membership or affiliation category and agree to observe the terms stated above.

CONSENT AND ACCEPTANCE
